Purpose of Collection:
We collect your personal information so that we can:
- Identify you when you do business with us
- Assess, accept and manage your claim
- Validate your claim
- Utilize the generic group information to negotiate with the Group Term Life Insurer
Consequences of not providing personal information:
We will not be able to process your claim or negotiate Group Term Life Insurance on your behalf.
When collected information will be disclosed:
- Your personal information is only disclosed by SNA for purposes directly related to the "Purpose of Collection".
- Generic D.O.B. and Classification are given to the Insurer as part of negotiating Group Term Life Insurance.
- If a Policy is written, individual names, D.o.B and Classification are given to the Insurer.
- If a death occurs, that individual's details will be provided to the Insurer to satisfy the payout requirements of the policy.
- Macarthur Credit Union is provided with Individual EFT details for the purpose of Funding the Pool and paying claims.
We do not use personal details for marketing
We do not undertake Secondary Usage of your Information
